West County Health Centers is recruiting mission driven, clinically excellent Registered Nurses passionate about working with a diverse underserved population in scenic rural Sonoma County.

Our nurses are an integral part of the Care Team, working alongside a Medical Provider, Medical Assistant, and Front Office staff to assist patients in disease self-management, care coordination, care transition management and system navigation. You will invest in long-term relationships with a diverse group of patients over time to develop a trusting, healing environment for patients to thrive.

A staff dedicated to providing a comprehensive care delivery structure, will support you and your patients. A strong Mental Health team, made up of Psychiatry, Clinic Psychologist and Licensed Clinical Social Worker will assist with patients needing support managing mental health needs. A Patient Navigator trained in Active Listening and Motivational Interviewing provides a resource for patients interested in setting realistic, short-term self-management goals. Our Wellness Center works with patients who are interested in integrating complimentary modalities such as Homeopathy, Osteopathy, Acupuncture and herbal remedies into their health care approach. We collaborate with a number of regional health partners to provide high-end health education and addiction counseling on-site.

We have strategically positioned West County Health Centers on the leading edge of Primary Care transformation toward In addition to RN Care Managers, we are also recruiting for a Clinic Nurse Manager position at one of our Primary Care sites. This Nurse Manager should have a passion for leadership and system development within the context of caring for an underserved population alongside an effective Management Team.

A Patient Centered Health Home, and advanced Nurse Care Management is at the heart of this transformation. We are convinced that it is the future of healthcare – because it is the right thing for our patients.

Further we have invested in the necessary resources to support the Nurse Care Managers to provide a high-end service for our patients. Our Medical Provider to RN Care Manager ratio is 1:1 per Full Time Equivalent. Our nurses receive training in Complex Case Management, Motivational Interviewing, use of evidence clinical guidelines, and management of hospital transitions. Additionally, we have a mature Care Team system in which all members of the team work at the top of their license using developed team-based workflows - allowing each member of the team to focus on what they are trained to do.
